Section 3: Document Upload*
Using this form upload the following:
  1. A tax return in the name of the parent or guardian from the most recent tax year that shows the student was claimed as a dependent.

  2. A document (in the student’s name) which verifies the student’s lawful presence in the United States.
    • Most common lawful presence documents include birth certificate, US Passport or GA driver’s license
    • Permanent residents must have a current Permanent Resident card on file in order to receive tuition at the out-of-state or in-state rates

  3. One document (in the name of the parent or guardian whose name appears on the tax return) from the approved list below:
    • Georgia State Income Tax Form 500
    • Georgia Automobile Registration
    • Georgia Voter Registration
    • Georgia Driver’s License (issue date must be at least 12 months before the first day of term)
    • Proof of ownership of primary residence in Georgia (date on document must be at least 12 months before the first day of term)
    • Notarized letter on company letterhead indicating lease status (lease date must be at least 12 months before the first day of term)
    • Utility bill from at least 12 months ago (example: electricity, cable, and water) - note that mobile phone bills are not accepted
