Satisfactory Academic Progress Appeal for Financial Aid Reinstatement

Federal regulations require the Office of Student Financial Aid to monitor the academic progress towards completing a program of study for students receiving
financial aid. For this reason, your Satisfactory Academic Progress (SAP) for financial aid is calculated at the end of each term to verify that you have met
all federal SAP standards. If you have failed to meet the SAP standards and feel you have mitigating circumstances, you may complete this appeal form and
provide substantiating documentation. If you are required to complete Parts 1 and 2 of this appeal form and do not provide substantiating documentation, your
appeal will automatically be denied.
If the Financial Aid Appeals Committee approves your appeal,
you will be required to accept the Terms and Conditions of the appeal via CTC student email before funds are applied to your account.
The Financial Aid Appeals Committee decisions are FINAL.

Part 1: Identify ALL terms in which you did not meet SAP standards (withdrew from class(es) or received grades
less than a C). Also, for each semester/course, please explain the reason(s) for not meeting SAP standards.

Part 2: Regarding what you listed above in Part 1, please explain how your circumstances have changed that led you to
your failure to meet SAP standards. Use as much space as you feel you need.

NOTE: Documentation is required if you completed Part 1 and 2. Please do not include transcripts as supporting documentation. |

Part 3: (Maximum Time Frame Suspension only) If you failed to meet SAP because you exceeded 150% of the attempted credit hours
required to complete your program, please explain why you need additional time to complete your program and feel you should be eligible
for financial aid. All periods of enrollment must be considered - even those for which you may not have received financial aid and any hours transferred
to CTC from another school.
